Please bring your insurance card, copay, medication list, referral information, and any useful prior test results. Our hours are Monday, Tuesday, Thursday and Friday 7:30am–5:30pm. We are closed Wednesdays, weekends and holidays. For after-hours concerns, please follow the instructions provided by our office. Access your child’s information through Children’s Health MyChart.
